Your expert for XERF treatment

XERF represents a new generation of non-invasive skin-tightening treatments. The system combines two monopolar radiofrequency frequencies to deliver controlled heat at different tissue depths. This can help tighten existing collagen fibres while stimulating the production of new collagen. Over time, the skin may appear firmer, and facial contours can become more defined.

The technology is currently being introduced to the European market. As a specialist in dermatology and aesthetic medicine with more than 30 years of experience, I am among the early adopters in Munich and Zurich to offer XERF as part of my treatment portfolio. For me, the deciding factor is not simply how new a device is, but whether the treatment is suitable for your skin, your anatomy, and your individual aesthetic goals.

A XERF treatment may be an option for patients experiencing early signs of skin laxity, reduced firmness, or less defined facial contours. As no incisions or injections are required, the procedure can generally be integrated easily into everyday life with minimal disruption.

During a personal consultation at my practice in Munich, I will assess your skin and discuss the results you can realistically expect. I will also advise you if an alternative treatment option may be better suited to your individual needs and aesthetic concerns.

What is XERF and how does the treatment work?

XERF is a non-invasive skin-tightening treatment that uses monopolar radiofrequency energy. Unlike laser-based procedures, XERF does not rely on light energy. Instead, radiofrequency energy encounters resistance within the tissue, where it is converted into heat.

What makes XERF unique is its combination of two radiofrequency frequencies: 6.78 MHz and 2 MHz. Depending on the selected settings, energy can be delivered to superficial, intermediate, or deeper tissue layers. This allows the treatment to be tailored to the characteristics of the treatment area and the desired aesthetic outcome.

The controlled heating process can cause existing collagen fibres to contract, creating an immediate tightening effect. At the same time, it stimulates the body’s natural regenerative response, encouraging the production of new collagen and the remodelling of existing collagen structures over the following weeks and months. This process is known as collagen remodelling.

XERF also features advanced energy delivery technology, integrated cooling, and continuous skin-surface temperature monitoring. These mechanisms help protect the outer layers of the skin while allowing the heat required for collagen stimulation to develop within the deeper tissue.

It is important, however, to maintain realistic expectations. XERF is not a substitute for a surgical facelift and cannot fully correct severe skin laxity or significantly sagging tissue. The treatment is primarily intended for individuals seeking a moderate, natural-looking tightening effect and who prefer to avoid surgery.

Differences compared with other skin-tightening treatments

Unlike other skin-tightening technologies that rely on either a single radiofrequency frequency or focused ultrasound, XERF combines two monopolar radiofrequency frequencies. This dual-frequency technology allows superficial, intermediate, and deeper tissue layers to be heated in a controlled manner during the same treatment session. While all of these technologies aim to improve skin firmness through collagen stimulation, they differ in how and where energy is delivered within the tissue:

  • Thermage: Also uses monopolar radiofrequency technology, but operates with a single frequency.
  • Ultherapy: Utilises micro-focused ultrasound to deliver thermal energy precisely into predefined tissue layers.
  • Ultraformer MPT: Uses focused ultrasound with different cartridges designed to target treatment depths ranging from 1.5 to 13 millimetres.

The most suitable treatment depends on several factors, including skin quality, the area being treated, and the desired outcome. At present, there is no conclusive evidence demonstrating a general superiority of XERF over other established skin-tightening technologies.

Who is a suitable candidate for XERF treatment?

XERF may be an appropriate option if your skin shows early to moderate signs of laxity. Common concerns that can be addressed include reduced firmness in the mid and lower face, softer facial contours, the appearance of early jowls, or a less defined jawline.

The treatment may also be suitable if you are looking to proactively support and maintain skin firmness before more pronounced ageing changes occur. Rather than age alone, the key factor is the individual condition of the skin and underlying tissue.

During a consultation, your skin quality, facial anatomy, and treatment goals are carefully assessed to determine whether XERF is the most appropriate option for achieving a natural-looking result.

Areas of application

Treatment parameters are individually tailored to each area being treated. Potential applications include:

  • Forehead and brows: The treatment can help improve skin firmness in the upper face and contribute to a more open, refreshed eye appearance.
  • Eye area: Fine lines and early skin laxity around the eyes may be treated, provided the individual findings are suitable.
  • Cheeks: Collagen remodelling can help improve tissue firmness and create a tighter appearance in the mid-face.
  • Lower face: Treatment may be considered for reduced definition and the early development of jowls.
  • Jawline: The aim is to create a clearer distinction between the face and neck for improved contour definition.
  • Under-chin area: Loose tissue can be tightened, helping to create a more balanced and defined facial profile.
  • Neck: Radiofrequency treatment may improve skin firmness and reduce the appearance of fine lines.
  • Décolletage: In suitable candidates, the skin texture can be firmed and smoothed.
  • Selected body areas: Whether treatment is appropriate depends on the area, skin thickness, and the degree of tissue laxity.

Early clinical observations have also indicated potential improvements in several of these treatment areas. In a retrospective study on XERF treatment published in August 2026, 16 female patients were evaluated following a single treatment session. The authors reported improvements in skin laxity, brow and upper eyelid position, as well as overall skin quality.

However, these findings should be regarded as preliminary. Due to the small number of participants, the retrospective study design, and the absence of a controlled comparison group, larger prospective and controlled studies are required to reliably assess treatment efficacy and long-term outcomes.

Importantly, the available data do not support the conclusion that XERF is superior to other monopolar radiofrequency technologies, such as Thermage, or to alternative non-invasive skin-tightening procedures.

It is also important to understand the limitations of non-invasive treatments. In cases of more advanced skin laxity or significant excess skin, the potential for improvement may be limited. During your consultation, I will provide an honest assessment of whether XERF is likely to achieve your goals or whether an alternative treatment approach may be more appropriate for your individual situation.

Procedure

How the XERF treatment is performed

Doctor-Duve-Aesthetische-Beratung

Consultation

Before the treatment, I will discuss with you the specific concerns you would like to address and the results you hope to achieve. I will then assess your skin quality, underlying tissue, and the anatomical characteristics of the area to be treated.

It is important to me that you have realistic expectations about what XERF can achieve. While the treatment can help improve skin firmness and support more defined contours, it does not produce the same results as a surgical lifting procedure.

I will also explain the potential risks, alternative treatment options, and the expected timeline of results. Only after this comprehensive consultation will we determine together whether XERF is the most suitable treatment for your individual needs and goals.

Preparation

Ideally, your skin should be free of make-up and not irritated on the day of treatment. It is advisable to avoid intense sun exposure, tanning beds, and highly irritating cosmetic procedures in the days leading up to your appointment.

Please inform me about any medications you take regularly and whether you have any implants located in or near the treatment area. You should not discontinue prescribed medication without consulting your treating physician.

During your consultation, I will review all relevant medical information to ensure that XERF treatment can be performed safely and is appropriate for your individual situation.

Treatment

At the beginning of the procedure, I cleanse the skin and carefully define the treatment areas. The XERF handpiece is then systematically moved across the selected regions, delivering controlled radiofrequency energy in a precise and consistent manner.

Depending on the thickness of the skin, the treatment area, and your individual goals, I adjust both the depth and intensity of the energy. During the procedure, you will gradually experience a warming sensation. An integrated cooling system helps protect the skin surface and enhances overall comfort throughout the treatment.

The treatment session typically takes approximately 30 to 60 minutes, depending on the size and number of areas being treated. In most cases, no anaesthesia is required.

Aftercare

Following treatment, the skin may appear temporarily red, slightly swollen, or feel warmer than usual. These reactions are generally mild and typically subside within a short period. You can leave the clinic immediately after the session and, in most cases, resume your normal daily activities without significant downtime.

On the day of treatment, you should avoid excessive heat and anything that may further irritate the skin. This includes saunas, very hot baths, intense sun exposure, and strenuous physical exercise. In addition, it is important to use an appropriate sunscreen and follow a gentle skincare routine to support the recovery process.

Your skin will continue to undergo collagen remodelling in the weeks and months following treatment. As a result, improvements in skin firmness and contour definition usually develop gradually over time. During your follow-up appointments, your progress can be assessed and any further treatment recommendations discussed.

Are there any side effects associated with XERF treatment?

XERF is a non-invasive procedure. Nevertheless, it is a medical aesthetic treatment that uses intensive radiofrequency energy. For this reason, careful patient selection and individually tailored treatment settings are essential. When performed by an experienced physician and with appropriate treatment parameters, serious complications are generally not expected.

Possible temporary side effects may include:

  • Redness
  • Mild swelling
  • A sensation of warmth or tightness
  • Tenderness or sensitivity to touch
  • Temporary tingling or numbness
  • In rare cases, minor bruising

Before treatment, I will explain all potential side effects and any individual risk factors in detail, ensuring that you are fully informed before making a decision.

XERF treatment costs

The cost of XERF treatment depends on several factors, including the areas being treated, the size of those areas, and the extent of treatment required for your individual concerns. As a general guideline, prices start at €1,500.

Following a personal consultation and assessment, I will provide you with a transparent treatment and cost plan tailored to your needs. This allows you to understand the expected costs before treatment begins.

As XERF is considered an aesthetic procedure, the costs are typically not covered by statutory or private health insurance providers.

Häufige Fragen zu XERF

Answers from expert Dr Duve

What is XERF treatment?

XERF is a non-invasive skin-tightening treatment that uses monopolar radiofrequency energy. The controlled heating of different tissue depths stimulates the remodelling and production of collagen, helping to improve skin firmness and support more defined contours over time.

What does “dual frequency” mean in XERF?

Dual Frequency bezeichnet die Kombination der beiden Radiofrequenzen 6,78 MHz und 2 MHz. Damit kann die Energie oberflächliche, mittlere und tiefere Gewebebereiche erreichen.

Which areas can be treated with XERF?

The areas most commonly treated include the forehead, brow area, cheeks, lower face, jawline, under-chin area, neck, and décolletage. The suitability of selected body areas depends on skin thickness, tissue structure, and the degree of skin laxity.

Is XERF treatment painful?

Usually not. During the delivery of radiofrequency energy, patients typically experience a noticeable warming sensation, while the integrated cooling system helps protect the skin surface and generally makes anaesthesia unnecessary.

How long does a XERF treatment take?

A XERF treatment typically takes between 30 and 60 minutes, depending on the number and size of the areas being treated. A comprehensive treatment of the face and neck generally requires more time than the treatment of a single area.

When are the first results visible?

An initial tightening effect may be noticeable immediately after treatment. However, the full results develop gradually over the course of several weeks to months, as the stimulated collagen remodelling process takes place over time.

How long do the results of XERF treatment last?

The longevity of the results varies from person to person and depends on factors such as skin quality, age, lifestyle, and the natural ageing process. If the effects gradually diminish over time, a maintenance treatment may be considered following a medical assessment.

How many XERF treatments are necessary?

A single treatment session is often performed initially, followed by an observation period of several weeks to assess the treatment response. Whether additional treatments are necessary depends on the initial findings, the treatment goals, and the individual’s response to the procedure.

What experience has been reported with XERF treatment?

Is the effectiveness of XERF supported by scientific research?

Yes, although the current body of scientific evidence is still limited. A retrospective study published in 2026 involving 16 female patients reported improvements in several parameters following a single treatment session. However, due to the small sample size and the lack of a control group, the findings should be regarded as preliminary.

Is XERF better than Thermage?

No, XERF has not been proven to be superior to Thermage. Both technologies use monopolar radiofrequency, with XERF combining two frequencies, while Thermage operates using a single frequency.

What is the difference between XERF and Ultherapy?

XERF uses monopolar radiofrequency technology to heat targeted tissue layers. Ultherapy, by contrast, uses micro-focused ultrasound to deliver thermal energy precisely to predetermined tissue depths.

What is the difference between XERF and Ultraformer MPT?

XERF uses two monopolar radiofrequency frequencies to provide controlled heating across multiple tissue layers. Ultraformer MPT, by contrast, uses focused ultrasound with interchangeable cartridges that target treatment depths ranging from 1.5 to 13 millimetres.

When should XERF not be used?

XERF should not be used during pregnancy or breastfeeding, or in the presence of active skin infections or open wounds within the treatment area. Certain electronic or metallic implants, as well as specific pre-existing medical conditions, may also make the treatment unsuitable.

How much does XERF treatment cost in Munich?

The cost depends on the number, location, and size of the areas being treated. For this reason, a binding price can only be provided once the individual treatment plan and scope of treatment have been determined.
